Diabetes Self-Management Education (DSME)
DSME is a service where health care providers educate patients to successfully self-manage diabetes. The program includes instruction in self-monitoring of blood glucose; education about diet and exercise; an insulin treatment plan developed specifically for the patient who is insulin-dependent; and motivation for patients to use self-management skills. Medicare and private insurance payers cover DSME services when these services are furnished by an accredited provider (practitioner or organization) that meets certain quality standards. Our clinical pharmacists provide a robust diabetic self-management education program as well as a diabetes prevention program for eligible patients. Our classes range from 30 minutes to 60 minute sessions, with options for 1-on-1 counseling sessions or group therapy sessions. We can also provide our diabetes education sessions via tele-health to accomodate any patients that may not be able to travel to the pharmacy for in-person sessions.
