Medication therapy management (MTM)

Medication therapy management (MTM) is a group of services that optimize therapeutic outcomes for individual patients. The 5 core elements of MTM include Medication Therapy Review, Personal Medication Record (PMR), Medication Action Plan (MAP), Intervention & Referral, Documentation & Follow-Up. These elements are provided through Comprehensive Medication Reviews (CMR), Targeted Medication Reviews (TMR), and Therapeutic Intervention Programs (TIP). Clinical Pharmacists will provide MTM services to help patients get the optimum benefits from their medications by actively managing drug therapies and identifying and resolving drug therapy problems. Our MTM programs include the following documented elements: review of the pertinent patient history, medication profile (prescription and nonprescription), and recommendations for improving health outcomes and treatment compliance. Patients eligible for a CMR will be seen by a pharmacist who will review their medications for drug interactions, therapeutic duplication, high cost medications, and provide clinical recommendations to their health care provider. We provide in-person / virtual / telephonic consultations to tailor our MTM programs to the specific needs of the patient/partner we work with.
